Instructions for Completing the Aquatic Incident Debrief Form post emergency
Complete this form as soon as possible after the incident while details are fresh in your mind, ideally within 24 hours.
Fill in all sections completely. If information is unknown, write "UNKNOWN" rather than leaving blank spaces.
Be factual and objective - record only what you observed or verified, not assumptions.
For times, use 24-hour clock format (e.g., 14:30 instead of 2:30 PM).
In the sequence of events, provide clear, chronological details. Include timestamps where possible.
For the Response Evaluation section, check all actions taken and provide specific times. This information is crucial for improving future responses.
When documenting "What Went Well" and "Areas for Improvement," be specific and focus on actionable points.
Collect contact information from all witnesses before they leave the scene if possible.
Take photos of any relevant equipment or environmental conditions if safe and appropriate to do so. Note these in Additional Notes.
Have your supervisor review the completed form before filing.
Remember, this document may be used for legal purposes - accuracy and completeness are essential.
